If you would like to request a loaner HotSpot for your student please fill out the form by clicking on the link. We ask that you submit one form for each student that will be needing access. Please try to enter your address the same way for every student that lives in your household.

If you have the ability to print the permission form please do. This will speed up the process greatly. If you can't a form will be provided for your student that must be signed and returned before the hotspot can go home.
